1. CSPM, its agents or representatives and any third party contracted by CSPM agrees not to disclose any privileged or confidential information provided by the Member to any individual or entity, except as where necessary to perform credentialing services, required by state and federal laws and regulations or upon written authorization from the Member.
3. Telephone numbers and email addresses are used to contact you the member regarding your membership. We will never sell, rent, share or give your personal information to anyone without your express permission, or unless required by law.
4. Upon enrolling in our RM membership you will be required to provide demographic information to CSPM. This information includes but is not limited to: Name, mailing address, telephone number including cell number, date of birth, social security number, npi number, state(s) medical license(s), federal DEA.
5. At no time will you be required to provide this information via the cspmllc.com website. Within 24 hours of processing your membership payment you will receive an email from our enrollment team at firstname.lastname@example.org instructing you on how and were to submit the required information and documentation.
7. All certification and documentation are maintained in a secured system that is not accessible on line for added security measured. However CSPM cannot be liable for disclosures of your personal information due to errors in transmission, unauthorized acts of third parties, security breaches of vendors contracted by CSPM.
8. Please note CSPM Consultants, LLC reserves the right to transfer your personal information and other information maintained by CSPM as an asset in connection with a proposed or actual merger or sale.
9. If you elect to use our referral campaign for informing colleagues of our services, we ask you for your colleagues name and email addresses. CSPM will automatically send a one-time email to your list of referrals inviting them to visit our site. CSPM will store this information for the sole purpose of sending this one time email and tracking the success of our referral campaign.
